| Accumed provides complete medical/clinical laboratory services to patients, physician practices, long-term care facilities, health departments and corporations. For patients whose doctors are requesting blood testing or specimen collection, we maintain several walk-in patient service centers. For physician practices, long-term care facilities, health departments and corporations, Accumed provides on-site phlebotomy, specimen courier service, laboratory analysis and result delivery. |

If I am a patient in need of lab work: |

| Do I need a prescription from my doctor? |
|
| Yes. New Jersey law requires that all patients in need of lab work must have a written prescription from their doctor. |

| Do I need an appointment to come to Accumed’s walk-in centers? |
|
| No appointments are necessary. |

| Where do my lab results go? |
|
| All lab results are sent directly to your ordering doctor immediately upon completion of testing. Copies can be forwarded to additional doctors upon request. |

| Do I need to fast or take any other steps to prepare for my bloodwork? |
|
| Yes, on occasion. Certain lab tests do require fasting and other preparations. If unsure, contact Accumed prior to arriving at the lab. Please call (888) 303-8700. |

| Can I take my medications if my lab tests require fasting? |
|
| Please check with your doctor regarding any medications. |

| What is the cost for lab tests? |
|
| For patients without insurance, Accumed maintains fair and reasonable fees for each test ordered by your doctor. For patients with insurance coverage, Accumed will submit all charges directly to your insurance carrier. You will only receive a bill for deductibles, and patient responsibilities. |
